Medical Marijuana Evaluation for Ky Medical Cannabis Program

• Any type or form of cancer
• Chronic or severe pain
• Epilepsy or any other intractable seizure disorder
• Multiple sclerosis
• Muscle spasms or spasticity
• Chronic nausea or cyclical vomiting syndrome that has proven resistant to other conventional medical treatments
• Post-traumatic stress disorder
• Glaucoma
• A terminal illness

Have medical records ready to upload or bring to your visit. These records need to support the qualifying condition you are seeking medical cannabis treatment for. If your medical condition has not been formally diagnosed, our provider may diagnose, but additional expenses will apply to diagnostic fees, tests, etc. 

Call our office at 606-669-6040 to schedule your appointment. The first visit must be in-person at our office on Frontier Blvd in Stanford, Ky. 

A $50 deposit will be required to hold your spot. It is non-refundable and will be applied as a fee for appointments not canceled or rescheduled with at least 24 hours' notice. 

You will meet with a medical provider authorized by Kentucky for the Medical Cannabis Program. The visit takes 20-45 minutes. If you are qualified for the program, the provider will create a treatment plan and submit your written authorization to the state registry. 

Once you have a written authorization in the system, you have 60 days to submit your application to the www.kymedcan.ky.gov website/portal to obtain your certification card. The cost to the state is $25 and the application could take up to 30 days to process. 

If approved, your certification is good for 1 year. 

You may renew your certification annually by scheduling a visit with our provider again, in person or via telehealth, and repeating the process. 

The medical cannabis evaluation provided by KyMedCan Clinic does not guarantee approval for medical cannabis use. A licensed medical professional conducts the evaluation to determine if a patient qualifies under Kentucky state regulations.
